Before you order your electric wheelchair scooter, please fill out the Medicare qualification form and fax to 415-508-1083 or email to info@citywheelchairs.com. A special form called the certificate of medical necessity needs to be filled out and signed by your doctor before we can deliver your electric wheelchair scooter. We have all the forms; all we need is some general information about yourself and your doctor. Once the forms are filled out we will contact you about the different kinds of electric wheelchair scooter units that Medicare will pay for. City wheelchairs will bill Medicare and any secondary insurance for you and we handle all other paperwork as well; so there are no worries regarding your electric wheelchair scooter.
